Bay and Basin Community Resources Ltd in partnership with Exsitu Pty Ltd, the University of Wollongong, and Carers NSW Ltd has been awarded an ARIIA grant for their ‘Regional Provider Blueprint for Developing a Values-Based Culture and Values-Driven Care Plans through Engagement, Empowerment and Technology’ project.

One in three older Australians live in regional and remote areas of Australia, with up to 50% of people over the age of 85 living alone. This project addresses the unique challenges faced by ageing people living within regional areas. Our focus is on connecting communities and empowering individuals to help alleviate loneliness, reduce social isolation, and improve mental health and wellbeing by creating opportunities to connect with others and have a sense of ‘belonging’.  According to the World Health Organisation, social isolation and loneliness has serious impacts on older people’s physical and mental health, and their quality of life.

BCR Communities and BCR Communities Online creates opportunities for social connectedness within a regional setting. Lifestyle activities will be driven by participant values and interests, encouraging our broader (eligible) community members to connect to aged care services and influence the services that we deliver.

Community members will start their journey to a more empowered and individualised aged care experience through ExSitu, a values-based innovative technology solution that generates individualised care plans. These care plans form the basis of each person’s values-based supports delivered by a frontline workforce trained in values-driven care, supporting empowerment and self-efficacy to achieve wellness and reablement.
