Preparing for Support at Home

Participant one used the exchange to learn about Support at Home delivery for First Nations Elders.

The challenge

Participant one reported having limited information and support about Support at Home. Elders faced challenges navigating the system and bureaucratic delays while waiting for services.

The organisation was also responding to growing demand for in-home care. Complex processes were affecting service delivery and placing strain on funding for Elders’ in-home care programs.

What the exchange focused on

Participant one wanted to understand Support at Home processes, challenges and ways to manage them as delivered by the host organisation who had been undertaking this care model successfully for a longer period of time. The exchange also aimed to build relationships, observe practice in real time and learn from a similar organisation.

What stood out:

  • service delivery processes
  • budget tracking and maintaining Elders’ package budgets
  • working with Services Australia
  • claiming for service delivery
  • administrative, technical and financial capability
  • navigating My Aged Care
  • people processes and software systems

They used observation, knowledge sharing and training during the exchange. The team also discussed scenarios with its in-house care management team to consider local needs.

Planned local action

The participant identified several next steps:

  • develop an in-house program manual and process flowchart
  • train staff
  • run pilot activities
  • prepare administrative resources and configure software
  • continue buddying with the exchange partner during Support at Home implementation

The learning was shared with the management team. The participant said success would be seen when Elders started accessing services through My Aged Care.

Plan, learn and keep moving

The participant recommended starting with a clear plan for what the exchange should achieve. It also highlighted the value of remaining open to learning from peers and maintaining momentum after the visit.
